Londonderry Dining Services is
committed to ensuring that students with disabilities, allergies, and intolerances are
provided the same opportunities to participate in the school meals program as
all other students.
If your child requires special meals or has a meal
restriction (allergy), please have a physician fill out the Special Meals Prescription Form
and return it to the dining services department. Please note that a generic doctor's note is not in alignment with the Department of Agriculture's mandated paperwork.
This form must be completed annually to ensure that your child has access to a special meal program and/or the allergy/intolerance is monitored.
Please allow for up to four weeks for dining to process a form so as to ensure the allergy/intolerance/special meals are organized and our staff is trained to monitor your child's specific needs. You will be alerted by either electronic mail, paper mailing, or telephone call that your child has been accepted into our program and he or she may join us for meals.
There is also a list of
frequently asked questions surrounding the Special Meals Prescription form and the national program.
Please contact the school nurse, your child’s 504 coordinator, or the director of dining services, with any further questions or
concerns.
Note: All of Londonderry Dining Services run "nut conscious" kitchens. We do not cook with any peanut or tree nut products. At the elementary school sites, all products are peanut and tree nut free. At the high school and middle school some of our packaged foods do contain nut products. We do not guarantee that our products at any level are from nut free facilities.
